All questions must reflect the applicant's personal information. Only one form per couple is required. Applicant agrees to the rules and conditions set forth by the Milwaukee Islamic Dawah Center and council with a Shiekh/ Imam is required before Nikkah. Failure to meet our conditions will result in your request being denied.
